I have been using 1Password for a while now and find it very easy to use and organize. You can include notes with anything you enter.
It's a paid service but for $5 per month, I think it's worth making my life much easier. Take a guess at how many password the average computer user handles daily?

We are online hoarders
We’re creating and virtually stashing more online account information than ever. The average Inbox Scan user has over 107 accounts registered to one email address – an insanely high number of accounts to manage. In the US and UK, this figure rose to 130 and 118 respectively.
